Interventional cardiology procedures increasingly require auxiliary diagnostic screening, so that the type and extent of the lesion can be better assessed before stent placement. IVUS catheters fulfill this need by providing images of the lesion from inside the artery. These catheters are generally used during angioplasties for screening the target lesion for stent placement confirmation, likelihood of restenosis and plaque characteristics.

The transducer attached to the IVUS catheter is able to provide black-and-white tri-dimensional images of different layers of the lumen, even allowing the screening of calcified areas of the vein, where stenting can be difficult. These images are generated by a process of reverberation, where sound waves created by the transducer echo back, thus enabling this information to be converted into pictures on a monitor.

In addition to pre-dilation screening, IVUS catheters can also aid cardiologists in post-stenting examination. One of the advantages of using an IVUS catheter is that the device allows the visualization of the stent after its deployment. In this situation, the IVUS catheter is capable of checking if the stent matched the diameter of the lesion, decreasing the likelihood of a thrombosis caused by partial stent expansion, which is a rising concern amongst physicians.

In addition, IVUS can be used for detecting stent fracture and other post stenting complications. The penetration rate of IVUS is still growing despite the introduction of a competing imaging modality, optical coherence tomography (OCT). Competition from OCT will limit sales of both IVUS catheters and consoles, however, it is not likely to decrease the penetration rate of IVUS by any substantial measure because many cath labs will choose to have both IVUS and OCT consoles, especially those consoles that also have fractional flow reserve (FFR) functionality.
