Improving Blood Flow to the Brain in Arteries With Plaque Did Not Improve Cognitive Skills

Research Highlights:

  • Procedures that remove plaque from a narrowed carotid artery in the neck or prop the artery open with a stent did not appear to be better at improving cognitive function than medications and lifestyle changes.

“Whether patients undergo a procedure to remove plaque in the carotid artery (carotid endarterectomy), stenting to insert a flexible tube to hold open the narrowed part of the artery, or a combination of medications and lifestyle guidance without a procedure, there should be no expectation that cognition will improve after the treatment,” said study lead author Ronald M. Lazar, Ph.D., FAHA, a professor of neurology and neurobiology at the University of Alabama at Birmingham (UAB) and director of the UAB McKnight Brain Institute.

Carotid artery stenosis, a narrowing of the carotid arteries in the neck, is usually due to plaque buildup. Standard treatment involves intensive medical management, including aspirin to prevent blood clots, medication for high blood pressure or cholesterol, and lifestyle changes such as smoking cessation, weight loss if needed, increased physical activity and a healthy diet. In addition, when the artery is more than 70% blocked, either carotid endarterectomy or stenting may be recommended.

The CREST-2 (Carotid Revascularization and Medical Management for Asymptomatic Carotid Stenosis) Trial compared stroke rates among people randomly selected to receive either intensive medical management alone; intensive management combined with carotid endarterectomy; or intensive management plus stenting.

Almost all previous research has found an association between carotid stenosis and lower performance on tests of cognitive function. The CREST-2 substudy included a ”cognitive core” component to assess cognitive performance before treatment and yearly for up to four years. This is the first large carotid stenosis study to include cognition as a major outcome in the study design of a randomized controlled trial.

“The brain needs a steady flow of blood to get oxygen for its cells. When blood vessels can’t deliver enough blood, the brain doesn’t get enough oxygen, and the neurons can’t work properly, which impacts the brain’s ability to function properly. This often leads to changes in cognitive skills like attention, processing speed and decision-making skills,” Lazar said.

Lazar and coauthors published results in 2021 in Stroke, the peer-reviewed scientific journal of the American Stroke Association, a division of the American Heart Association. The study showed that, before carotid stenosis treatment, 786 patients with severe (over 70% blocked) but symptomless carotid stenosis who entered the CREST-2 trial had lower cognitive skills scores - especially in memory - compared to participants in a separate population-based study matched for similar demographics and cardiovascular risk factors.

“The 2021 analysis indicated that revascularization might improve cognitive function,” said Lazar, who is the principal investigator of the Cognitive Core substudy and overall co-investigator of CREST-2.

However, with an average of 2.8 years of follow-up after treatment, the CREST-2 substudy researchers found no difference in thinking or memory over time among participants who had a procedure to restore blood flow (stenting or surgery) and those treated solely with intensive medical therapy. “Even among participants with the lowest cognitive function at the start of the study, who were expected to gain the most from these treatments, there were still no differences in cognitive skills among the treatment groups,” Lazar said.

Cognitive decline was confirmed in the participants who had a stroke during the study, validating that the cognitive tests were able to detect real changes in brain and neurological function.

These results may change how health care professionals counsel patients about the benefits of stenting or surgery to open narrowed carotid arteries. “Health care professionals can no longer assert that treatment of carotid stenosis will improve cognition. However, worsening cognition over time may be a signal that treatment may need to be re-evaluated and possibly adjusted,” Lazar said.

The study could not determine whether reduced blood flow to the brain is the sole or most important reason for cognitive decline in carotid artery disease. “Some characteristics of a blockage can cause small particles to travel to the brain. These particles may, over time, affect how the brain functions. This is an area we plan to explore in our future research,” Lazar said.

The study’s additional limitations include that all cognitive tests were conducted over the phone, making it impossible to test participants’ visuo-spatial skills and the full range of executive functions, such as decision-making. Also, only English-speaking patients were included in the substudy, so, the results may not be generalizable to other populations.

“These results from CREST-2 do not provide evidence of benefit of carotid revascularization on cognitive function among patients with significant carotid stenosis, although there is a benefit for stroke reduction,” said Mitchell Elkind, M.D., M.S., FAHA, FAAN, the American Heart Association’s Chief Science Officer for Brain Health and Stroke and a past volunteer president of the Association (2020-2021). “Cognitive decline associated with aging is a complex problem; however, restoration of blood flow through the large vessels alone may not be sufficient to address the many other pathways to decline, such as inflammation, neurodegeneration and small vessel disease. More research on how to mitigate cognitive decline and reduce dementia risk is needed, which is why the American Heart Association has supported study of these important areas and others.”
