"Unfortunately, we have seen patients here at the clinic to correct problems from unqualified injectors," said Parveen Sangha, a licensed nurse practitioner and the medical director of Promethean Clinic, in a recent interview with CBC News.

Sangha brings seven years of family medicine experience and previously served as Medical Director for Vancouver Coastal Health programs and as President of the NP Council for the Nurses and Nurse Practitioners of BC. "They come with a lot of risks – scary risks," Sangha told CBC. "If they are injected incorrectly, it could go into an artery, which can cause a blockage and lead to tissue death, permanent blindness, infection and scarring in some of the worst scenarios."

The accessibility of social media has created an unregulated marketplace for aesthetic procedures. Unlicensed practitioners advertise services including dermal fillers, neurotoxins, and body contouring treatments through Instagram and other platforms, often using professional-sounding titles that mislead consumers about their qualifications.

Recent legal actions by the College of Physicians and Surgeons of BC underscore the severity of the problem. Court documents detail cases where unlicensed providers used unapproved products and advised patients to obtain antibiotics under false pretenses, resulting in serious medical complications requiring hospital treatment.

According to the BC Ministry of Health, only specific regulated health professionals can legally administer cosmetic injectables: physicians regulated by the College of Physicians and Surgeons of BC, registered nurses regulated by the BC College of Nurses and Midwives, naturopathic doctors regulated by the College of Complementary Health Professionals of BC, and dentists for Botox only.

Sangha stressed that legitimate providers follow strict protocols that unlicensed operators typically skip.

"Licensed providers should offer clients a thorough consultation, where they review the individual's medical history, discuss possible complications and sign consent forms," she explained, nothing that as a Nurse Practitioner regulated by the BCCNM, her practice is held to the same diagnostic and prescribing standards as a physician.

Qualified practitioners must also be able to prescribe medications, properly assess and diagnose complications, and refer patients to specialists like plastic surgeons when necessary.

"Anyone can call themselves an expert injector on social media," Sangha noted.
